1. Please download the SAQ (Self-Assessment Questionnaire) form you have to fill out as follows.

     – If you have any troubles in selecting the SAQ or downloading it, please contact us at your
     convenience so that we can send it back to you.

2. Please refer to the link when you fill out the downloaded SAQ form.

   • Please type your IP address from your business place at "URL" of Part 1a on page 1.
     To get the IP address, please visit www.myipaddress.com on your computer at your business place.
   • If you need any further assistance for completing the form, please contact CDS PCI Support Team.

3. Once the form is complete, please send it back to us by email/fax/mail at your convenience.
   • Email: If you have a scanner, please scan the documents and send them back to pci@cdsus.com
              with attachment.
   • Fax: If you have a fax machine, please fax the documents to Fax: 1-888-560-4288
   • Mail: If you prefer to mail the form, please send the documents to the below address.

§ After you send the form, please make sure if it is well received. When it is, we’ll send a confirmation email to you. Please keep the confirmation email just in case.

   If you need to download the ‘Security Policy’ or update it due to your environment changes
   such as hiring new employees for the past year, then please Click here to download!
   • Type your business name in DBA on it and keep this document in your business place.
